Insurance & New Client Paperwork

 We are in network with all Blue cross/Blue shield plans (Highmark Blue Shield, Capital Blue Cross, Independence Blue Cross, Anthem, Horizon) and Aetna. We are out of network for all other plans.

We also provide services for cyberschools and individual school districts, including Commonwealth Academy Cyber School.

New Client Paperwork

We are using an Electronic Medical Records system called JANE where families can access treatment notes and reports, view and pay their balance and complete initial and update forms. Once you have scheduled an appointment, you will get an email from Jane with a link to complete initial paperwork.
